The initial evaluation with Dr. Whitfield is 60-90 minutes in length.  He will collect medical history information and spend time getting to know you and your specific concerns.  Dr. Whitfield will develop a treatment plan that may include psychotherapy and medication management.  Because healthcare is a collaborative process, Dr. Whitfield will coordinate his care with your other providers.  

If medications are indicated, you will be required to attend follow-up appointments for medication management.  Refills will not be authorized without an appointment.  

Dr. Whitfield is an out of network provider.  Please contact your insurance company if you need to verify your benefits.  Payment is collected at the time of service.  If you are canceling or rescheduling an appointment, we ask that you do so at least 24 hours in advance.

Before your first appointment, our office will send you a packet of forms to be completed before you arrive for your appointment.  By doing this before you arrive, Dr. Whitfield can offer you a more comprehensive examination.
